Unja Smart Clinic as part of a health service institution plays an important role in ensuring the quality of services provided to patients. Clinic accreditation is one of the mainindicators for assessing the quality of these services. In the accreditation process, one aspect of concern is patient screening, which includes recording initial health data to support diagnosis and treatment. However, the manual system currently used often causes delays and data discrepancies, which can affect accreditation assessments. This research aims to design an integrated technology-based patient screening information system, to increase the efficiency of the screening process while providing accurate and easily accessible data for accreditation evaluation. This system was designed using a User-Centered Design (UCD) based software development method to ensure that the needs of users, such as medical staff and patients, aremet optimally. The system's key features include digital recording of screening data, integration with electronic medical records, as well as analytical reports to support the accreditation process. The design results show that the proposed information system is able to speed up the screening process, minimize data input errors, and provide accurate reports that comply with accreditation standards. By implementing this system, Unja Smart Clinic is expected to be able to improve service quality, operational efficiency and better meet accreditation standards.
